If you spot a possible violation and wish to file a complaint, contact Florida DBPR. <\/p>\n<h2>Palm Beach County Restaurant Closures<\/h2>\n<h3>Aroma Indian Cuisine<\/h3>\n<p>731 Village Blvd., Suite 110, West Palm Beach<br \/>\n<strong>Ordered shut:<\/strong> June 4<br \/>\n<strong>Why<\/strong>: Nine violations (five high-priority), including:<\/p>\n<ul>\n<li>About 12 live roaches in kitchen crawling \u201cinside oven on cook line,\u201d \u201cunder prep table with microwave,\u201d \u201con wall next to cooking equipment,\u201d \u201con wall next to triple sink over cooking utensils\u201d and \u201con shelving over prep table.\u201d<\/li>\n<li>About 18 dead roaches in kitchen in and around a \u201chand-wash sink at expo line,\u201d \u201cstuck to walls on cook line over food cooking\/preparation\/storage areas,\u201d \u201cin left-side, flip-top cooler on cook line inside pans containing raw shell eggs\/cooked cauliflower\/cooked chicken\u201d (stop sale ordered).<\/li>\n<li>\u201cEmployee handled menus and order entry system and then handled clean glasses, alcohol jigger and ice scoop while preparing drinks for customers without washing hands.\u201d<\/li>\n<li>\u201cEmployee touched visibly soiled wiping cloth and then handled clean utensils and plates while preparing food for customers without washing hands.\u201d<br \/>\n<strong>Status:<\/strong> Closed June 5 after a follow-up inspection found four violations (two high-priority) and also on June 6 when a third visit yielded two violations (one high-priority). Reopened later that day, with a follow-up inspection required, after a fourth visit found one intermediate violation.<\/li>\n<\/ul>\n<h3>Brick Oven Pizza<\/h3>\n<p>726 Lake Ave., Lake Worth Beach<br \/>\n<strong>Ordered shut:<\/strong> June 4<br \/>\n<strong>Why:<\/strong> Two high-priority violations, including:<\/p>\n<ul>\n<li>More than 23 rodent droppings \u201cat bottom of pizza station in kitchen\u201d and \u201con top of dish machine.\u201d<\/li>\n<li>\u201cRaw chicken stored over raw beef, not all commercially packaged,\u201d in reach-in freezer.<br \/>\n<strong>Status:<\/strong> Closed June 5 after a follow-up inspection found one high-priority violation. Reopened June 6 after a third visit found zero violations.<\/li>\n<\/ul>\n<h3>La Luna Italian Bistro &amp; Bar<\/h3>\n<p>5030 Champion Blvd., Boca Raton<br \/>\n<strong>Ordered shut:<\/strong> June 4<br \/>\n<strong>Why:<\/strong> Three violations (two high-priority), including:<\/p>\n<ul>\n<li>About 34 rodent droppings \u201con cans in dry storage area,\u201d \u201con floor in dry storage area away from kitchen\u201d and \u201con electrical panel in dry storage area away from kitchen.\u201d<\/li>\n<li>One live roach \u201con floor in front of tall reach-in freezer near salad prep cooler.\u201d<\/li>\n<li>\u201cHood filters soiled with grease and food debris.\u201d<br \/>\n<strong>Status:<\/strong> Reopened June 5 after a follow-up inspection found no violations.<\/li>\n<\/ul>\n<h3>Brothers Taquizas food truck<\/h3>\n<p>1600 N. Dixie Highway, West Palm Beach<br \/>\n<strong>Ordered shut:<\/strong> June 2<br \/>\n<strong>Why:<\/strong> 14 violations (12 high-priority), including:<\/p>\n<ul>\n<li>Four live flies \u201cflying around in food truck\u201d and \u201clanding on single-service box that was placed on counter to serve customers.\u201d<\/li>\n<li>Stop sale ordered \u201cdue to temperature abuse\u201d for items such as cooked rice, cooked peppers, raw ground beef, cooked chicken, cooked pork, cooked beef, raw shrimp and red salsa.<\/li>\n<li>Stop sale ordered for tortilla \u201cstored on visibly soiled wiping cloth\u201d inside a cooler (\u201cnon-food grade towel used as liner for food container\u201d).<\/li>\n<li>\u201cObserved employee handle soiled dishes and chlorine at triple sink then put gloves on and began to take cooked chicken from container in cooler to reheat on flat-top grill\u201d without washing hands.<\/li>\n<li>\u201cObserved employee wash hands with water from hose at triple sink with no soap then [handle] clean utensil.\u201d<\/li>\n<li>\u201cObserved no running water or triple sink area. Operator has a hose from outside with a handle to spray, hanging on the side of the triple sink.\u201d<br \/>\n<strong>Status:<\/strong> Reopened June 5 after a second visit found two intermediate violations, with a follow-up inspection required.<\/li>\n<\/ul>\n<h3>Elle Caribbean Grill<\/h3>\n<p>220 N. Congress Ave., Boynton Beach<br \/>\n<strong>Ordered shut:<\/strong> June 2<br \/>\n<strong>Why:<\/strong> 25 violations (seven high-priority), including:<\/p>\n<ul>\n<li>Nine flies seen landing \u201con triple sink,\u201d \u201con inspector\u2019s shoulder,\u201d \u201con soiled apron across slicer,\u201d \u201con slicer,\u201d \u201con lid of the chest freezer across cook line\u201d and \u201con pastry prep table, while prepping [was] in progress.\u201d<\/li>\n<li>Stop sale ordered for beef empanadas \u201cdue to temperature abuse.\u201d<\/li>\n<li>Food debris spotted in the microwave, as well as on the \u201cbottom inside of reach-in freezer at cook line\u201d and on a flour container.<\/li>\n<li>\u201cIce machine used for drink ice unprotected from contamination. Dry wall work conducted around ice machine.\u201d<\/li>\n<li>\u201cEmployee personal charger, keys, cellphone and glasses stored in or above a food preparation area, next to slicer.\u201d<\/li>\n<li>\u201cEmployee washed hands with no soap after picking up food debris on the floor.\u201d<\/li>\n<li>\u201cCook picked up food debris off the floor and kept on preparing food at cook line without changing gloves and washing hands.\u201d<\/li>\n<li>All cooked food prepared the previous day and stored in a walk-in cooler had no date mark.<br \/>\n<strong>Status:<\/strong> Reopened June 3 after a second inspection found one intermediate violation; a follow-up inspection was required.<\/li>\n<\/ul>\n<h2>Broward County Restaurant Closures<\/h2>\n<h3>Express Caribbean Restaurant<\/h3>\n<p>1401 S. State Road 7, North Lauderdale<br \/>\n<strong>Ordered shut:<\/strong> June 4<br \/>\n<strong>Why:<\/strong> 18 violations (four high-priority), including:<\/p>\n<ul>\n<li>About 40 rodent droppings \u201con storage shelf underneath bag of jasmine rice,\u201d \u201con the floor in storage room off kitchen,\u201d \u201cin mop sink next to three-compartment sink,\u201d \u201con shelves on cook line,\u201d \u201con the floor next to freezer chests in back storage room\u201d and \u201con the floor in front of two-door glass beverage cooler at front counter.\u201d<\/li>\n<li>Stop sale ordered for pikliz (\u201cdue to temperature abuse\u201d) and cooked turkey (time\/temperature issue).<\/li>\n<li>Time\/temperature issue also involved cooked pork \u201csitting out at room temperature in kitchen.\u201d<\/li>\n<li>\u201cObserved employee cellphone on prep table.\u201d<\/li>\n<li>\u201cObserved can of Hot Shot ant and roach spray on top shelf in kitchen.\u201d<br \/>\n<strong>Status:<\/strong> Reopened June 5 after a second inspection found four basic violations.<\/li>\n<\/ul>\n<h3>McDonald\u2019s<\/h3>\n<p>10901 W.
