Alexis Osmani Gonzalez-Companioni, 27, was arrested after the June 28 crash. An indictment filed Friday charged him with five counts of manslaughter and four counts of aggravated assault with a deadly weapon.

Company Official Indicted for Forgery

A company official with Hope Trans LLC was also indicted Friday for third-degree felony forgery. The official, who has yet to be publicly named pending the issuance of an arrest warrant, is charged with falsifying the “cab card” — the proof of registration for the semitractor-trailer Gonzalez-Companioni was driving at the time of the accident.

The crash occurred near Terrell about 2:40 p.m. While driving a semitrailer, Gonzalez-Companioni fell asleep and struck a Ford F-150, which was carrying a Fort Worth family, police said. That crash caused a pileup, as two other semis and at least three other vehicles were struck during the crash.

Four of the F-150’s occupants were killed, while the fifth was sent to the ICU. The members of the family who died were identified as 52-year-old Zabar McKellar, 45-year-old Krishaun McKellar, 16-year-old Kason McKellar and 79-year-old Billy McKellar.

Nicole Gregory, 49, of Dallas was also killed in the crash.

The National Transportation Safety Board announced it is investigating the crash.

Gonzalez-Companioni remained in the Kaufman County jail Friday on bail totaling $2.25 million. His attorney was not immediately able to be reached for comment.
